We have an opportunity for a Pathways and EPR Lead to join our team of staff.

This is a Full Time role for 37.5 hours a week. Role will be based at Head Office – London.

Applicants should meet the following criteria:

  • Nursing or AHP qualification and registration
  • Evidence of continuing professional development.
  • Knowledge of patient pathways
  • Knowledge of EPR systems
  • Master equivalent of health-related qualification
  • Able to manage multiple projects and deadlines.
  • Use of process mapping tools and software
  • Lean Six sigma or equivalent skills and training
  • Proficient IT skills

Duties of this role include:

  • The post holder will act as the clinical lead co-ordinator on Patient Pathways development across the business and the development of Electronic Patient Record across the Circle Health Group (CHG).
  • They will have a detailed understanding of key specialities and variation in provision and best practice models
  • Ensure that EPR development is patient focused and supports sae and patient centred care
  • Horizon scan and bring to the table ideas on innovation and improvements in patient pathways
  • Interface with non-clinical colleagues and drive integrated patient pathway developments
